With over 15 years of experience as a school psychologist, Ms. Kokulak is well versed in a variety of  assessment components.  These include, but are not limited to, cognitive (IQ) testing, academic achievement assessment, social emotional assessment, executive functioning assessment, adaptive behavior assessment, autism behavior assessments (NOT ADOS assessment )and social developmental history.

What can you expect?

In a private evaluation,  I am able to provide unbiased opinions regarding a student's functioning; that cannot always be attained in the school setting and known staff.  WIthin just a few sessions you will be provided with a comprehensive evaluation that is tailored to meet your child's assessment needs.  Typically the first session is  the clinical/parent/guardian interview and social developmental history.  Assessment times vary depending on  child and evaluation components necessary.

How much does a private evaluation cost?

Just as your child is different from  others, each evaluation will vary, depending on the assessment components, and number of respondents.  Inquire today to discuss your needs.  Pricing is as such:

Assessment behavioral observation, comprehensive written report and 30 minute consultation post evaluation included in pricing.

Insurance is NOT accepted. Payment is accepted via credit card or social payment services (Zelle, etc.) 

$200.00 NON-REFUNDABLE DEPOSIT IS REQUIRED TO SECURE AN APPOINTMENT. 

Evaluation plan will be constructed at the time of interview.  Consent for evaluation and a 50% of total evaluation estimated cost will be collected at that time.  Interviews can be in person or virtual.
